Przedmiotem zamówienia jest dostawa leków do Apteki Szpitalnej w Rzeszowie z podziałem na 2 zadania ( 2 części). Szczegółowy opis przedmiotu zamówienia znajduje się w formularza cenowych stanowiących załącznik nr 3 do specyfikacji warunków zamówienia. 1. Przedmiot zamówienia został podzielony na 2 zadania (części). 2 Przedmiot zamówienia obejmuje również dostawę przedmiotu zamówienia transportem wykonawcy, na jego koszt i ryzyko. 3. Przedmiot zamówienia winien spełniać odpowiednie warunki dopuszczenia do obrotu - leki winny być zarejestrowane, wpisane do Rejestru Środków Farmaceutycznych. 4 Zamawiający dopuszcza składanie ofert równoważnych. Za ofertę równoważną Zamawiający będzie uważał preparat leczniczy (synonim) posiadający identyczną dawkę substancji czynnej, postać farmaceutyczną, pojemność, drogę i sposób podawania oraz wskazania do stosowania, jak określony przez Zamawiającego. Zadanie obejmuje dostawę 2 pozycji asortymentowych; Zadanie obejmuje dostawę 1 pozycji asortymentowej
