Description

1. Przedmiotem zamówienia jest DOSTAWA SPRZĘTU MEDYCZNEGO JEDNORAZOWEGO UŻYTKU dla Wojewódzkiego Szpitala Specjalistycznego im. NMP w Częstochowie. 2. Opis przedmiotu zamówienia został zawarty w formularzach asortymentowo-cenowych, stanowiących Załącznik Nr 2 do SWZ. Pozostałe warunki dotyczące realizacji zamówienia zostały określone w projektowanych postanowieniach umowy (PPU), stanowiących Załącznik Nr 6 do SWZ. 3. Przedmiot zamówienia podzielony jest na 20 części, tj.: Część nr 1 - IGŁY SPECJALISTYCZNE I Część nr 2 - IGŁY SPECJALISTYCZNE II Część nr 3 - IGŁY SPECJALISTYCZNE III Część nr 4 - IGŁY DO BIOPSJI Część nr 5 - DRENY REDONA, POJEMNIKI DO DRENAŻU RAN, ZGŁĘBNIKI ŻOŁĄDKOWE Część nr 6 - CEWNIKI CENTRALNE Część nr 7 - CEWNIKI CENTRALNE ANTYBAKTERYJNE Część nr 8 - CEWNIKI DO EMBOLEKTOMII, DREN BRZUSZNY Część nr 9 - RURKI TRACHEOSTOMIJNE BEZ MANKIETU I Z MANKIETEM Część nr 10 - ZGŁEBNIK GASTROSTOMIJNY TYPU G-TUBE CH14, CH18, CH20 Część nr 11 - MYJKI JEDNORAZOWEGO UŻYTKU Część nr 12 - DROBNY SPRZĘT MEDYCZNY Część nr 13 - MASKA OCHRONNA TYP FFP3 Część nr 14 - GLUKOMETRY I PASKI DO GLUKOMETRÓW Część nr 15 - ŁĄCZNIKI SCHODKOWE Część nr 16 - ŁĄCZNIKI Y Część nr 17 - ZESTAW RATUNKOWY DO KRIKOTYROIDOTOMII (KONIKOTOMII) METODĄ CHIRURGICZNĄ Część nr 18 - ZESTAW DO NABŁONKOWEGO DRENAŻU PĘCHERZA MOCZOWEGO DLA DZIECI Część nr 19 - FARTUCH FOLIOWY OCHRONNY Część nr 20 - FARTUCH CHIRURGICZNY, STERYLNY 4. Zamawiający dopuszcza składanie ofert częściowych. Pod pojęciem oferty częściowej rozumie się pojedyncze części ustalone w Załączniku Nr 2 do SWZ. Wykonawca może złożyć ofertę na dowolne części lub na cały przedmiot zamówienia. 5. Kod CPV: 33140000-3 - materiały medyczne 33199000-1 - Odzież medyczna Przedmiot zamówienia został szczegółowo opisany w ilościach i asortymencie w formularzu asortymentowo-cenowym, stanowiącym Załącznik nr 2 do SWZ.
