Description

Przedmiotem zamówienia jest dostawa materiałów neurochirurgicznych. Szczegółowy opis przedmiotu zamówienia zawiera załącznik nr 1a do SWZ. Opis ten należy odczytywać wraz z ewentualnymi zmianami treści SWZ, będącymi np. wynikiem udzielonych odpowiedzi na zapytania Wykonawców. 1. Zestaw stymulatora DBS z elektrodami kierunkowymi (szczegółowy opis zawarty w arkuszu cenowym) 1.1. generator do głebokiej stymulacji mózgu 2 kanałowy - 30 szt. 1.2. elektroda domózgowa 8 kontaktowa- kierunkowa - 20 szt. 1.3. łączniki rozciągliwy 50cm/60cm/90cm - 20 szt. 1.4. pierścień do mocowania elektrody - 20 szt. 1.5. programator pacjenta - 15 szt. 1.6. elektroda do microrecordingu - 40 szt. 1.7. kaniula do microrecordingu - 40 szt. 2. Zestaw stymulatora DBS z możliwością doładowywania baterii przez pacjenta (szczegółowy opis zawarty w arkuszu cenowym) 2.1. generator 2 kanałowy ładowalny - 6 szt. 2.2. łączniki- adapter do łacznika - 12 szt. 2.3. programator pacjenta przewodowy - 6 szt. 2.4. ładowarka - 6 szt. 2.5. akcesoria do ładowarki - 6 szt. 3. Stymulator rdzeniowy przeciwbólowy (szczegółowy opis zawarty w arkuszu cenowym) 3.1. Generator - 1 szt. 3.2. Elektroda 20 kontaktowa lub 2x elektora przezskórna - 1 szt. 3.3. Łącznik (zestaw) - 1 szt. 3.4. Programator pacjenta - 1 szt. 3.5. Jałowa głowica do stymulatora próbnego lub kabel do stymulacji śródoperacyjnej - 1 szt.; 1. Igły jednorazowe do EMG - 615 szt.
