1. Przedmiotem zamówienia są: IMPLANTY NEUROCHIRURGICZNE dla Wojewódzkiego Szpitala Specjalistycznego im. NMP w Częstochowie. 2. Opis przedmiotu zamówienia został zawarty w formularzach asortymentowo-cenowych, stanowiących Załącznik Nr 2 do SWZ. Pozostałe warunki dotyczące realizacji zamówienia zostały określone w Załączniku Nr 6 do SWZ, stanowiącym projektowane postanowienia umowy (PPU). 3. Zamawiający dopuszcza składanie ofert częściowych. Pod pojęciem oferty częściowej rozumie się pojedyncze części ustalone w Załączniku Nr 2 do SWZ. Wykonawca może złożyć ofertę na dowolne części lub na cały przedmiot zamówienia. 4. Przedmiot zamówienia obejmuje 3 część tj.: CZĘŚĆ NR 1 - Protezy trzonów kręgowych odcinka szyjnego - PEEK CZĘŚĆ NR 2 - Protezy trzonów kręgowych odcinka szyjnego - TYTAN CZĘŚĆ NR 3 - Protezy trzonów kręgowych odcinka szyjnego 5.
