Description

1) Przedmiotem Zamówienia jest dostawa, instalacja i uruchomienie 1 szt. aparatu USG klasy Premium wraz ze szkoleniem. W ramach niniejszego Przedmiotu Zamówienia należy uwzględnić sprzęt, dostawę sprzętu do Zamawiającego, instalację, uruchomienie sprzętu, podłączenie sprzętu do istniejącego w Szpitalu systemu RIS/PACS oraz szkolenie personelu. 2) Zamawiający nie dopuszcza składania ofert częściowych. Zamówienie nie zostało podzielone na części. Wykonawca możne złożyć tylko jedną ofertę. 3) Szczegółowy opis Przedmiotu Zamówienia zawiera Załącznik 4, będący integralną częścią SWZ. Opis ten należy odczytywać wraz z ewentualnymi zmianami treści SWZ, będącymi np. wynikiem udzielonych odpowiedzi na zapytania Wykonawców. 4) Oferowany sprzęt musi być wyrobem medycznym dopuszczonym do obrotu i używania na terenie Polski zgodnie z postanowieniami ustawy z dnia 07.04.2022 r. o wyrobach medycznych oraz z rozporządzeniem Parlamentu Europejskiego i Rady (UE) 2017/745 z dnia 5.04.2017 r. w sprawie wyrobów medycznych. 5) Oferowany sprzęt musi być fabrycznie nowy (rok produkcji: 2026, nieużywany, kompletny i do jego uruchomienia oraz stosowania zgodnie z przeznaczeniem nie jest konieczny zakup dodatkowych elementów i akcesoriów. Aparat ani żadna jego część składowa, wyposażenie, etc. nie może być sprzętem rekondycjonowanym, powystawowym lub wykorzystywanym wcześniej przez innego użytkownika. 6) Wymagany termin płatności wynosi do 30 dni. Szczegółowy Opis Przedmiotu Zamówienia zawiera Załącznik nr 4 do SWZ
