Вищевказана довідка про походження має бути адресована замовнику та містити наступну інформацію : реквізити особи, яка видала довідку, найменування та кількість одиниць товару, що постачається, назва виробника, країна виробника, країна походження товару, ПІБ особи, яка видала довідку та її підпис скріплений печаткою
