Description

1. Het bieden van (somatische) huisartsenzorg, waarvoor geen reguliere financiering beschikbaar is via hetzij de ZVW, WLZ, CAK of rechtstreeks van de patiënt, aan daklozen en thuislozen, die verblijven in de dag- en nachtopvang van Maatschappelijke Opvang Den Bosch, cliënten van het CvTB en cliënten bekend bij de GGD; 2. Het bieden van (somatische) huisartsenzorg aan cliënten met een eigen huisarts waarvoor de drempel te hoog is, waarna zij naar de eigen huisarts worden toegeleid; 3. Het diagnosticeren van daklozen en thuislozen, en zo nodig doorverwijzen naar specialistische medische zorg; 4. Het outreachend werken om de medische zorg voor deze patiënten te verbeteren; Naar schatting €520.000 voor 4 jaar.
