На виконання п.57 Постанови №822 від 14 вересня 2020 р. з метою підтвердження, що країною-походження товару не є Російська Федерація/Республіка Білорусь/Ісламська Республіка Іран, Учасник, пропозиція якого за результатами оцінки визначена економічно вигідною електронною системою закупівель, протягом двох робочих днів повинен надати скановану копію оригіналу документу про походження товару, отриманого від виробника або офіційного представника, або дилера, або дистриб’ютора, уповноваженого виробником на розповсюдження його продукції на території України, на запропонований товар. Документ повинен містити: ідентифікатор закупівлі - номер оголошення, оприлюдненого на веб-порталі Уповноваженого органу; повну назву Постачальника; назву предмету закупівлі, країну походження товару та повинен адресуватися Покупцеві. Надані довідки становлять невід’ємну частину цього Договору. Поставка Товарів здійснюється Постачальником партіями за потребою згідно заявки Покупця (надісланої засобами електронного, телефонного або факсимільного зв’язку), не пізніше 3-х робочих днів після отримання заявки. Місце поставки (передачі) товарів: КНП «Кіровоградська обласна лікарня Кіровоградської обласної ради», 25030, м. Кропивницький, проспект Університетський,2/5. З підняттям товару на 7-й поверх
