1. Przedmiotem zamówienia jest DOSTAWA MATERIAŁÓW OPATRUNKOWYCH, OCHRONNYCH I HIGIENICZNYCH WYROBÓW J. U., ZESTAWÓW OBŁOŻENIOWYCH J. U. dla Wojewódzkiego Szpitala Specjalistycznego im. NMP w Częstochowie. 2. Opis przedmiotu zamówienia został zawarty w formularzach asortymentowo-cenowych, stanowiących Załącznik Nr 2 do SWZ. Pozostałe warunki dotyczące realizacji zamówienia zostały określone w projektowanych postanowieniach umowy (PPU), stanowiących Załącznik Nr 6 do SWZ. 3.
