Description

1. Przedmiotem zamówienia jest DOSTAWA MATERIAŁÓW OPATRUNKOWYCH, OCHRONNYCH I HIGIENICZNYCH WYROBÓW J. U., ZESTAWÓW OBŁOŻENIOWYCH J. U. dla Wojewódzkiego Szpitala Specjalistycznego im. NMP w Częstochowie. 2. Opis przedmiotu zamówienia został zawarty w formularzach asortymentowo-cenowych, stanowiących Załącznik Nr 2 do SWZ. Pozostałe warunki dotyczące realizacji zamówienia zostały określone w projektowanych postanowieniach umowy (PPU), stanowiących Załącznik Nr 6 do SWZ. 3. CZĘŚĆ NR 26 - LIGNINA CELULOZOWA CZĘŚĆ NR 27 - OPATRUNEK DO ZABEZPIECZENIA CEWNIKÓW CZASOWYCH I PERMANENTNYCH CZĘŚĆ NR 28 - PRZYLEPNE PASKI DO ZAMYKANIA RAN CZĘŚĆ NR 29 - POKRYCIE HIGIENICZNE NIEPRZEMAKALNE NA ROLCE 50CM X 50 M 4. Zamawiający dopuszcza składanie ofert częściowych. Pod pojęciem oferty częściowej rozumie się pojedyncze części ustalone w Załączniku Nr 2 do SWZ. Wykonawca może złożyć ofertę na dowolne części lub na cały przedmiot zamówienia. 5. Kod CPV: 33140000-0 - Jednorazowe, niechemiczne artykuły medyczne i hematologiczne 33771200-7 - Pieluszki dla niemowląt 33771100-6 - Podpaski Przedmiot zamówienia został szczegółowo opisany w ilościach i asortymencie w formularzu asortymentowo-cenowym, stanowiącym Załącznik nr 2 do SWZ.
